New Service to Promote Health in Patients Aged 75 and Over
Along with other practices in South Warwickshire, we have recently launched a new service which aims to provide holistic clinical reviews for those aged 75 and over. The reviews will be conducted by our Nurse, Linda Sanins who will invite you to attend the review.
The purpose of the review is to undertake an in-depth assessment of the health status of the person being reviewed, with a view to considering what pro-active measures may be helpful in helping the person maintain good health. Additionally, for those who are more frail or who have poorer health we have teamed up with Age UK Warwickshire who will undertake a thorough assessment covering aspects such as ability to look after oneself on a daily basis, transport, financial wellbeing and safety. It is hoped that these comprehensive reviews will complement each other and we will be able to improve the care we provide to individuals, as well as signposting them onto other services and resources that can assist in living healthily and happily. The practice will be contacting patients individually throughout the year to offer them the opportunity to participate in this service.
The ‘over 75s project’ is a pilot running to 31st March 2016. We will be asking for feedback and encouraging comment from those who use the service – there is the potential for it to continue if we can show we are having a positive effect on the health and wellbeing of the people who use the service.
