Ordering Repeat Prescriptions online

The online form you may have been using to request your repeat medication via our website has now ceased.

We aim to make this process much easier for both yourself and the practice by using patient access via this link   You will need to register with the practice for on-line access if you have not already done so, all the necessary forms are available on the Online Services page of this website.  Please bring the form with two forms of identification (driving licence/passport and a utlity bill) into the practice to enable our reception staff to supply you with your login details between the hours of 10.00am and 5.00pm.

Once this has been done the process will be simple by registering on the patient access website.  You will then see your medication on the repeat medication screen and all you have to do is tick the medication you require.  This will go directly into our clinical software for the Drs to check and sign.

If your request is rejected you can see the reason why by logging into the patient access website and the reason will be on your medication screen.

These can only be obtained when authorised by your doctor. Please give 48 hours’ notice for the surgery to generate your request. The chemist may also need 24-48 hour's notice to dispense the medication. All patients having repeat prescriptions will be required to see their doctor at regular intervals.

By post: a stamped, addressed envelope must be included if you want the prescription returned to you by post

Please state if you are going away on holiday and when. Please allow 48 hours for processing and leave enough time to account for weekends and Bank Holidays when we are closed. This email is for repeat prescriptions only, please do not request any other information by email.

Electronic Prescription Service

The Electronic Prescription Service allows your prescription to be sent electronically to a pharmacy of your choice.

To sign up, you need to speak to your preferred pharmacy and complete a form to confirm that they can receive your prescription directly.

Once you have signed up, your chosen pharmacy will receive your prescription electronically and you will not have to pick up your paper prescription from us.

If you have already nominated a pharmacy, your prescription will be sent to them electronically unless you tell us otherwise. If you need to obtain a paper prescription for any reason, just let us know at the time of ordering. 

 The Chase Meadow Health Centre are taking part in a Pharmacy Waste Project it is supported by the CCG, a number of practices within South Warwickshire  are looking at ways in which we can reduce waste and make our patients more aware of the costs of their medication.
